Position Overview

We are seeking a Hospice RN Case Manager to join our dedicated team, providing comprehensive patient care in home and facility settings. This role involves assessing patient needs, coordinating care, and supporting families through their hospice journey.

Key Responsibilities:

• Conduct initial and ongoing comprehensive assessments of patients and families.

• Develop and implement individualized care plans based on patient needs.

• Administer medications and treatments as prescribed.

• Provide healthcare education to patients, families, and caregivers.

• Supervise hospice aides to ensure quality patient care.

• Assist patients with daily living activities and optimize comfort measures.

• Maintain accurate and timely clinical documentation.

• Collaborate with an interdisciplinary team to coordinate care.

Qualifications:

• Current RN license in the state of Illinois.

• CPR certification required.

• Bachelor’s degree preferred, with one year of hospice experience (preferred but not required).

• Strong assessment, communication, and problem-solving skills.

• Must have a valid driver’s license, reliable vehicle, and auto insurance.

• Willingness to travel within assigned service areas.
