About the Role

Oasis Hospice is seeking a Licensed Practical Nurse (LPN) to provide compassionate, hands-on care to patients in collaboration with our interdisciplinary team. Under the supervision of a Registered Nurse, you will play a crucial role in delivering high-quality end-of-life care, ensuring comfort and dignity for patients and their families.

Key Responsibilities:

• Provide direct patient care in accordance with the State Nurse Practice Act.

• Follow and implement the plan of care, ensuring accurate and timely documentation.

• Educate patients and families on care plans, symptom management, and hospice services.

• Coordinate with hospice aides, therapists, social workers, dietitians, and other team members.

• Ensure proper pain and symptom management, monitoring patient responses to treatments.

• Maintain compliance with infection control protocols and hospice regulations.

• Participate in ongoing training, quality improvement initiatives, and professional development.

Requirements:

• Valid LPN license in Illinois.

• Experience in hospice, palliative care, or home health (preferred).

• Strong clinical skills, documentation accuracy, and teamwork abilities.

• Compassionate, patient-centered approach to care.
