At Oasis Hospice & Palliative Care, we are committed to providing compassionate, high-quality hospice care to patients and families during life’s most challenging moments. Our Licensed Social Workers (LSWs) and Licensed Clinical Social Workers (LCSWs) play a vital role in providing emotional, psychosocial, and practical support to patients and their loved ones as they navigate end-of-life care.


Position Overview

We are seeking dedicated and compassionate social workers to join our interdisciplinary team. This role involves conducting psychosocial assessments, providing counseling, coordinating resources, and supporting families in coping with illness, grief, and loss.

LSWs focus on general case management, patient advocacy, and resource coordination.

LCSWs provide advanced therapeutic support, including individual and family counseling.


Key Responsibilities:

  • Assess patient and family needs to develop a personalized care plan.

  • Provide emotional support and crisis intervention for patients and caregivers.

  • Assist with advance care planning, end-of-life decisions, and legal/financial concerns.

  • Connect families to community resources, including financial assistance, housing, and counseling services.

  • Work with an interdisciplinary team (nurses, chaplains, therapists, physicians) to provide holistic care.

  • Facilitate grief and bereavement support for families.

  • Provide therapy and emotional counseling (LCSW-specific responsibility).

  • Maintain accurate and timely clinical documentation in compliance with hospice regulations.

  • Advocate for patient rights, dignity, and quality of life.


Qualifications:

  • Current Illinois LSW or LCSW license (required).

  • Master’s degree in Social Work (MSW) from an accredited institution.

  • Experience in hospice, palliative care, or healthcare (preferred but not required).

  • Strong counseling, advocacy, and problem-solving skills.

  • Ability to handle sensitive conversations with empathy and professionalism.

  • Must have a valid driver’s license, reliable transportation, and auto insurance.

  • Willingness to travel to patient homes and care facilities as needed.
